Our Annual Fee Plan for medical services not covered by OHIP provides a more convenient and cost effective way to manage extra charges.
About Our Annual Fee Plan
At The Medical Station, it is our goal to help you with all of your health needs and provide you with the best medical care in a friendly and efficient manner. The Ontario Health Insurance Plan (OHIP) pays for most of your medical visits to our office.
There are, however, uninsured services, which OHIP does not cover, such as prescription refills by phone, medical forms for insurance, legal or employment purposes, and photocopying, faxing and transferring medical records.
The Medical Station will charge for uninsured services in accordance with the fees guidelines set by the Ontario Medical Association (OMA) and the College of Physicians and Surgeons of Ontario (CPSO).
While you can pay for these services on an as-needed basis, Family Physicians at The Medical Station offer an Annual Fee Plan as a more convenient and cost-effective way to manage these charges. The Annual Fee Plan covers most of your uninsured services for the entire year for one annual fee. For the majority of patients this alternative to pay-per-service will provide significant financial saving.
To subscribe to this plan, please complete the Registration Form and submit it with your annual fee to our office. We accept VISA, MasterCard, and American Express. For your convenience, forms and credit card information may be faxed in (416) 633-2216 or phoned in (416) 633-2345.
